Dartmouth Health Connect is committed to helping our patients get access to COVID-19 updates as soon as possible. This page includes our most up to date information about COVID-19 vaccines.
As your primary care providers, we urge you to get the COVID-19 vaccine once it’s available to you. We have linked below the available COVID-19 vaccination websites in your area where you can schedule your shot. As you know, Iora does not have COVID-19 vaccines today. Please do not wait for Iora to get them!
We know that the process for scheduling COVID-19 vaccine appointments has been frustrating, time-consuming, and confusing. Unfortunately, the number of spots available depends on how many doses the state gets. The state also decides where the new doses are sent. All we can ask of you is to have patience and keep trying as more spots open up.

NEW HAMPSHIRE RESIDENTS:
- Currently, state-run vaccine sites are the only locations for those in Phase 1B.
- If you are 65 or older, register for your vaccine here or call 2-1-1 if you need help or have more questions.

If you are under 65 and have two health conditions listed by the state of NH – you may be eligible for phase 1B. If this includes you, we may be able to pre-register you with the state. Please contact us either through email: careteam@chirpcare.com or the patient portal: http://dhc.chirpcare.com/. Include the following information and allow us to share with the state:
- Full name
- Date of birth
- Email address and phone number
We will follow the NH DPHS process to verify your eligibility. We will review your chart and submit your information to the state if you meet medical requirements.
VERMONT RESIDENTS:
Vermont is currently vaccinating residents 65 and older. If you are 65 or older, register for your vaccine here.
Starting March 8, 2021
- Individuals aged 55+ with certain high-risk health conditions (VT is not requiring documentation from health care providers but will ask for provider contact information)
- K-12 teachers and onsite staff, child care providers, and expanded public safety works (VT is estimating 3/8/21 as a start date for this group depending on vaccine allocation)
- Register for your vaccine after the above date here
Starting March 15, 2021
- Individuals aged 16+ with certain high-risk health conditions (VT is not requiring documentation from health care providers but will ask for provider contact information)
