Joining the Practice

Who is eligible to join the practice?

Dartmouth Health Connect welcomes patients from Dartmouth College, King Arthur Flour, and adults 65+ on Medicare. If you have insurance from any of these organizations, you may be eligible to join our practice. Dartmouth Health Connect does not provide pediatric care and is only available to adults (18+).

I have preexisting medical conditions, can I still join the practice?

Yes. While everyone will benefit from our model of primary care, patients with chronic conditions may benefit the most. Our care teams are trained to empower patients with diabetes, heart disease, hypertension, asthma, COPD and other chronic diseases to manage their disease and improve their quality of life. We have been featured in the New Yorker article The Hot Spotters by Atul Gawande for our results helping patients with chronic disease dramatically improve their health status.

If I join the practice, does the rest of my family have to join as well?

There is no requirement for patients’ family members to join, but we welcome this if family members meet eligibility requirements. Remember, Dartmouth Health Connect is a primary care practice and not a health plan, so people become part of the practice on an individual basis. All Medicare members should be a member of one of the plans we accept including AARP Medicare Advantage plans from from UnitedHealthcare, Humana Medicare Advantage plans and traditional Medicare with – or without – most supplemental plans.
