How do I transfer my records?

If you would like to have prior medical records sent to us from a single medical provider sign a consent form allowing us to request them. To request records from multiple medical providers, complete this form instead. These can be brought with you to your first appointment or faxed to us at 603-653-8191.

Am I charged for lab tests at Dartmouth Health Connect?

At Dartmouth Health Connect we collect specimens onsite at no charge. While we are able to result some tests within the practice, if your specimen requires laboratory analysis outside of Dartmouth Health Connect, you will incur a fee according your insurance plan. The following lab tests will never incur a charge because we have the ability to result them onsite:

  • Finger-stick blood sugar test
  • Pregnancy test
  • Test for Strep Throat

What if my insurance changes?

Currently Dartmouth Health Connect is a benefit for those individuals who receive their health insurance through a partner organization.  Therefore, we are only able to accept patients who are eligible members of that organization. Should you need to change your insurance plan or obtain Medicare coverage and become ineligible under the current design, we will help you safely transition your care to a provider who is able to accept your new insurance.

What is your holiday schedule?

Regardless of holidays, we are available in the case of an urgent need at 603.738.1164. Dartmouth Health Connect observes the following holidays and our practice will be closed:

  • New Years Day
  • Martin Luther King, Jr. Day
  • Presidents Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Columbus Day
  • Thanksgiving Day
  • Day After Thanksgiving
  • Christmas Day
